A GAO review found that five selected VA medical facilities did not consistently follow VHA policy when reviewing and reporting providers with clinical care concerns from 2020–2024. GAO identified 104 providers with concerns and recommends improved training and stronger oversight to ensure consistent quality-of-care reviews and reporting.
